Case Manager
Job Description:
The candidate will:
1) Coordinate the patient care activities for the client.
2) Conduct clinical interviews with patients to assess a variety of patient needs.
3) Educate patient and family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
4) Perform patient telephone triage, assesses needs and coordinates needs with medical staff.
5) Communicate and interpret physician’s instructions to patients.
6) Suggest solutions to patient care crisis problems and complaints
7) Assist physicians in completing medical records concerning history, diagnosis, prognosis and current treatment plans. Maintains requried records.
Maintain established departmental policies and procedures.
9) Attend required meetings and participate in committees as requested.
10) Participate in professional development activities and maintains professional affiliations.
11) Maintain patient confidentiality.
12) Perform related work as required.
Qualifications:
A full time Case Manager position is available at Avenal Community Health Center. This position is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes. Graduate of an accredited school of nursing and two years of professional nursing experience. Possession of California Registered Nurse License. A Licensed Vocational Nurse will be acceptable as well. Current CPR certification. Bilingual English/Spanish required
Salary: $45,000 annually
