Job Description:
EXAMPLES OF DUTIES
This is a bilingual position which requires the ability to effectively communicate in both English and Spanish.
Duties may include, but are not limited to the following:
• Plans and participates in educational activities and training programs.
• Teaches health related classes; Dental Health, HIV, Tobacco, Basic Parenting and Daily Living Skills.
• Visits and interviews patients to assess living conditions/medical needs.
• Completes intake/assessments, including problem defined and issues to be resolved.
• Assists with the development of case plan and assists clients improving their personal situation.
• Counsels patients and provides referral assistance as needed.
• Monitors clients’ status as to services and referral outcome.
• May be required to translate for Non-English speaking clients.
• May provide Targeted Case Management services and engage in outreach and other activities to enhance services to Medi-Cal beneficiaries.
Qualifications:
MINIMUM QUALIFICATIONS
Experience:
Two (2) years of experience performing a variety of paraprofessional medical duties in a hospital, clinic, or community health agency under the supervision of licensed medical personnel.
OR
One (1) year performing duties equivalent to those of a Community Health Worker in Merced County.
Education:
Completion of college level course work in each of the following areas: Psychology or Communications: Physiology, including the concepts and treatments of communicable diseases; Anatomy and Biology.
License:
Possess and maintain a valid California Driver’s License.
Desirable:
Certificate in Phlebotomy and/or Medical Technology is desirable.

Salary: $2,977.87 - $3,620.93 Monthly
